Carol Bates was given it for setting up Crawley Old Girls Football Club back in 2015.
With five sessions throughout the week on the grounds of Crawley Town FC stadium, it’s for women who want to play football at any age.
Ms Bates was given the award at an event at Broadfield Stadium on Tuesday.
